ntracerebral hemorrhage (ICH) following evacuation of chronic subdural hematomas (CSDH) has been reported as a rare complication. We report 2 cases of ICH following evacuation of subdural hematomas. Rapid perioperative parenchymal shift may be suggested to cause direct vascular damage, sudden increase in intracerebral blood f l o w, and local disturbance of autoregulation. Slow decompression with gradual reexpansion of the brain parenchyma is recommended to avoid such complications. (Korean Journal of Stroke 2005;7(2): 247~249)
