Cholesterol crystal embolism is a serious complication of atheroma. Cholesterol embolism can give rise a various clinical spectrum, depending on the site of the embolism, but neurological manifestation involving spinal cord is extremely rare. A 68-year-old man visited hospital with both lower extremity weakness in a course of three days. He showed typical features of myelopathy and cyanotic skin lesions on the both big toe areas. Whole spine MRI showed no significant abnormality except the subtle focal high signal intensity lesion on T2-weighted images in high cervical areas. The kidney biopsy confirmed the presence of cholesterol cleft in the vascular space. Cholesterol embolism may present with myelopathy due to scattered crystal emboli.
