현미경적 혈뇨를 동반한 신동맥류에서의 경피적 코일 색전술 치료 1예

초록

신동맥류는 매우 드문 질환으로 통상 인구에서 0.09%의 빈도로 발견된다. 보통은 무증상이고 현미경적 혈뇨나 고혈압에 대한 검사 중 우연히 발견되는 것이 보통이다. 40∼50세에서 흔하며, 남자보다는 여자에서 그리고 왼쪽 보다는 우측 신동맥에 흔히 발생한다. 직경이 2.0 cm 이상 시에는 파열의 가능성이 있고 파열시 사망률이 10%에 이르기 때문에 적극적인 치료를 요한다. 저자들은 건강 검진에서 발견된 현미경적 혈뇨를 주소로 내원한 46세 남자에서 신장 초음파 등을 통해 혈류가 있는 2 cm 크기의 우측 신장 낭종성병변을 발견하였고, non- detachable platinum coil을 이용하여 성공적으로 색전술을 시행하였기에 보고하는 바이다.

Abstract

Renal artery aneurysms are relatively uncommon, being found in approximately 0.09% of the general population. They are usually found incidentally during investigation of patients with microscopic hematuria or high blood pressure. Rupture is the most lethal complication of renal aneurysms. Surgical treatment had been common for these aneurysms. We report a case of about 2-cm-diameter renal artery aneurysm successfully treated by embolization with non-detachable platinum coil.(Korean J Med 70:575-580, 2006)
