Background : In patients with internal carotid artery (ICA) occlusion, adequate cerebral blood flow is maintained by numerous collateral circulation that redistributes the cerebral blood to the flow-deficient area. The anterior communication artery (ACOM) and the posterior communicating artery (PCOM) are two of the main collateral circulations. The aim of this study is to evaluate the correlation between the patterns of collateral circulation and vasomotor reactivity in the patients with ICA occlusion. Methods : Thirty-five patients with ICA occlusion were studied retrospectively. Collateral pathways were assessed with cerebral angiogram and magnetic resonance angiogram (MRA). Acetazolamide (ATZ) challenged SPECT was used to measure vasomotor reactivity. Results : Our results illustrated that collateral pathway via ACOM showed a significant correlation with vasomotor reactivity detected by ATZ SPECT (P<0.05) whereas collateral pathway via PCOM did not. Conclusions : Collateral pathway through ACOM seems to have a main role in the maintenance of vascular reactivity in patients with ICA occlusion. Korean Journal of Stroke 2001;3(1): 76~78
