Nuclear oculomotor palsy associated with midbrain infarction may present with various combinations ofe x t r a o c u l a r, levator palpebrae, and pupillary sphincter muscle paralysis, depending on the three dimensionaltopography of the oculomotor nuclear complex. Typical signs of oculomotor nuclear lesion are ipsilateral 3rdnerve palsy plus contralateral levator palpebrae and supeiror rectus palsy. However, isolated extraocular musclepalsy has rarely been reported. In this paper, one patient showed typical ocular motor signs of the nuclear oculo-motor palsy and the other developed an isolated inferior recuts palsy from oculomotor nuclear lesion.
