Department of Neurology, Inje University, College of Medicine, Paik Institute for Clinical Reserch Hemiballism associated with stroke is not uncommon. However insular cortical infarction manifested by monoballistic movement is extremely rare. We experienced a case presenting with acutely developed ballistic movement of the left upper extremity caused by the insular cortical infarction. A brain MRI showed a high signal intensity lesion in the right insular cortex. Abnormal involuntary movement was well controlled with clonazepam. Insular infarction may be one of the causes of monoballism.
