외음부 섬유 상피성 용종 1예

섬유 상피성 용종은 주로 비뇨생식기계에 생기는 매우 드문 종양이다. 1966년에 Norris와 Taylor1가 처음으로 섬유 상피성 용종 12예를 보고한 이래로 문헌상 최근까지 100예 이하가 보고되었다. 그 기원은 확실하지 않으나 조직학적으로 교원질형, 점액성형, 혼합형, 비정형형의 네가지로 분류한다. 비정형형에서는 비정형성 기질세포와 세포분열의 비정형성이 관찰되므로 악성 종양과 조직학적으로 유사하여 가육종 또는 포도상 가육종으로 불리기도 하였다. 따라서 비정형형은 악성종양으로 오진될 가능성이 있다. 11세된 여자 환자가 3년전 부터 외음부에 무통성의 융기성 종물을 발견하였다. 종물은 좌측 외음부에서 융기되었으며 직경은 5×3 cm 이었다. 수술은 전신 마취하에서 절제술을 시행하였는데 종물은 주위의 피부와 그 밑의 진피와 전혀 경계가 뚜렷하지 않아 주위 조직들과 명백히 구분될 수는 없었으나 촉진으로는 구분이 되어 적출할 수 있었다. 촉지로써 전 종물을 제거한 후 피부 봉합을 하고 환자는 아무런 합병증 없이 수술 후 첫날에 퇴원하였다. 수술 1년 후 환자는 아무런 재발 증상없이 잘 지내고 있다. 현미경적 소견상 제거된 종물은 주로 소수의 비정형 단핵구와 다핵구 기질세포들과 콜라겐 다발들, 피부 부속기, 소수의 혈관들로 구성되어 있었다. 이들 세포들은 육종, 특히 포도상 횡문근육종으로 오진될 수 있다. 우리 저자들은 11세된 여자의 외음부에 생긴 섬유 상피성 용종을 경험하였기에 간단한 문헌 고찰과 함께 보고하는 바이다.
Abstract
Fibroepithelial polyp is a very rare tumor mostly arising at the urogenitalia. Norris and Taylor firstly reported 12 cases of fibroepithelial polyp in 19661 and until recently under 100 cases were reported in literature. A 11-year-old girl complainted the painless bulging mass at the left labium major. The bulging tumor was measured 5×3 cm in diameter. The margin of the mass was not demarcated ever from the skin and underlying dermis and the mass was not clearly differentiated from surrounding tissues grossly but could be delineated by palpation. The mass was excised by palpation under the general anesthesia. In microscopic finding it was primarily composed of stromal portion containing fibroblast-like stromal cells, with bundles of collagen, skin appendages, and small numbers of blood vessels. We report a case of fibroepithelial polyp of the vulva in a 11-year-old female with a brief review of the literature.
