삼음교 지압이 분만통증 및 자궁수축양상에 미치는 영향

목적: 본 연구는 분만에 대한 삼음교 지압의 영향을 알아보기 위해 분만과정 중 삼음교 지압을 시행한 후 분만통증의 변화, 분만소요시간, 혈중 내 stress hormone 농도 변화, 자궁수축의 변화를 평가하였다. 연구 방법: 분만 활성기시 삼음교 지압을 실시한 26명의 실험군 산모와 40명의 대조군 산모를 대상으로 하였다. 실험군은 산모의 자궁 경부가 3 cm 개대되었을 때, 산모의 삼음교를 연구자의 양쪽 엄지손가락을 이용하여 강압 (120 mmHg)으로 지압하였다. 실험군과 대조군의 분만 통증의 정도를 visual-analogue scale (VAS)를 이용하여 비교하였고, 분만 소요 시간, 자궁수축 양상, stress hormone (cortisol, epinephrine, norepinephrine)의 변화를 각각 비교하였다. 결과: 삼음교 지압을 시행한 군에서 유의한 분만 통증의 감소를 보였다 (p=0.007). 반면 분만 소요 시간은 두 군 사이에 차이를 보이지 않았으며 (487.5±215.4 and 434.6±203.4 mins), 혈중 호르몬의 변화의 차이 또한 관찰되지 않았다. 자궁 수축 검사 상, 자궁 수축의 강도, 빈도, 기간, 수축증가비율, 전체 수축 면적, 그리고 Montevideo unit 또한 두 군 사이에 변화량의 차이를 보이지 않았다. 결론: 진통 중 삼음교 지압은 산모의 분만통증을 유의하게 감소시켰다. 하지만 삼음교 지압의 통증감소의 기전은 혈중 stress 호르몬의 변화나 자궁수축 양상의 변화에 따른 결과로 설명할 수 없었다. 삼음교 지압의 분만통증 감소의 기전은 확실하지 않으나, 삼음교 지압은 분만통증의 감소를 목적으로 비용의 발생 없이 비침습적이며 손쉽게 시행할 수 있는 방법이라고 생각된다.
Abstract
Objective: To evaluate the effect of SP-6 acupressure on labor pain, labor duration, change in serum stress hormone levels, and the pattern of uterine contraction in laboring women. Methods: A total of 66 pregnant women were randomly divided into acupressure group (n=26) and control group (n=40). When cervical dilation reached 3 cm, acupressure group were received with the acupressure of 120 mmHg on the SP-6 points. As for the control group, thumbs were placed on the SP-6 points not applying any pressure. The severity of labor pain interpreted by visual- analogue scale (VAS), labor duration, serum stress hormone levels, and tocodynanometry findings were evaluated and compared between the two groups. Results: The severity of labor pain was significantly reduced in the acupressure group (p=0.007). Average labor durations in acupressure group and control groups were not significantly different (487.5±215.4 vs. 434.6±203.4 mins). Also, no significant differences regarding contraction duration, intensity, interval, rate of rise, area-under-curve, and Montevideo unit of uterine contraction were noted between the groups. No significant changes in the serum stress hormone level were noted in both groups. Conclusion: SP-6 acupressure effectively reduce perceived labor pain in pregnant women during active labor. However, the pain relief following acupressure were not attributable to modification of uterine contraction or changes in stress hormone levels. Though its mechanism is unknown, SP-6 acupressure seems to be a non-invasive, cost-free, and a simple method to reduce labor pain.
