희석 Vasopressin 대량 주입후 내시경하 자궁각절제술에 의한 난관간질부 임신의 치료

초록

=목적: 본 연구의 목적은 자궁간질부 임신에서 대량 희석 vaosopressin을 이용하여 내시경적 자궁각절제술이 효과적인 방법임을 보고자 한다.연구 방법: 2001년 6월부터 2004년 6월까지 본원 내시경센터에서 시행된 난관간질부 임신시 17명의 환자를 대상으로 하여 후향적 연구를 시행하였으며, 모두 대량희석 vasopressind을 사용하였다.결과: 자궁외 임신으로 진단받은 446예 중 17예 (3.81%). 이중 14예 (82.4%)는 파열되지 않았고 3예 (17.6%)는 파열된 난관간질부 임신이었다. 환자의 평균 연령 및 출산력은 35.6±3.9년 (범위 30-40세), 1.5±2.1 (범위 0-3)이었으며, 평균 무월경 기간은 54.5±9.6일이었고 초음파검사를 통해 측정한 평균 임신낭의 크기는 2.6×2.6 cm 이었다. 수술시 출혈량을 최소화하기 사용된 vasopressin의 양은 평균 190.0±55.5 cc 이었고, 절개 후 부위에 10명 (59%) endosuture로, 7명은 (41%)은 endoloop로 자궁각부위를 봉합하였고, 평균 실혈량은 18.0±16.0 cc, 평균 수술시간은 28.0±7.8분이었다. 혈청 β-hCG를 4일에서 60일까지 추적 관찰한 결과 15예 (88.2%)에서 4일 내에 90% 이상 유의하게 감소하였으며, 2예 (11.8%)에서는 60일 내에 90% 이상 유의하게 감소되었다.
