복강경하 전자궁절제술 후 발생한 직장-방광누공 1예

요로누공은 산부인과적 수술 후 발생하는 심각한 합병증 중의 하나이다. 요로누공 중 가장 흔한 형태는 방광-질누공이며, 드물게 방광-자궁누공, 요관-요관누공 등이 있다. 직장-방광누공은 매우 드물고 지금까지 산부인과적 수술 후 발생한 합병증으로 국내에 보고된 것은없다. 특히 골반내 종양, 대장의 염증성 질환 등이 없는 환자가 부인과 양성질환으로 자궁절제술을 받은 후 합병증으로 직장-방광누공이발생한 경우는 현재까지 영어 및 한글로 쓰여진 논문에서 보고된 적이 없다. 한편 직장-방광누공의 주 증상은 기뇨, 분뇨인데 반해 이 증례에서 환자는 항문을 통한 소변의 배출이 있었다. 이는 일반적인 직장-방광누공의 압력 방향을 거스르는 현상이기 때문에 잘 나타나지않는다. 저자들은 이와 같이 부인과 양성질환수술 후에는 거의 발생하지 않는 직장-방광누공을 진단하였으며, 항문을 통해 소변이 배출되었던 드문 증례를 단기 및 장기 합병증 없이 성공적으로 치료하였기에 이를 간단한 문헌 고찰과 함께 보고하는 바이다.
Abstract
Vesicovaginal fi stulas are a most common complication accompanied by gynecologic surgeries, but other types of fi stulas rarely occur. Especially rectovesical fi stulas arising post-operative complication occur in extremely lower possibility and very few cases have been reported in the world. Interestingly, our case showed a symptom of urinary drainage from the anus unlike other cases with pneumaturia or fecaluria. This symptom could hardly happen because urine is prevented from being released due to the internal pressure of the fi stula. We experienced and treated a fi rst case of rectovesical fi stula with a symptom of passing urine from the anus.
