(Transurethral Resection of the Prostate, TURP)의 안전성과 효과성을 비교·평가하고자 하였다.
Abstract
Objectives: This study aimed to compare and evaluate the safety and efficacy of prostatic artery embolization (PAE) and transurethral resection of the prostate (TURP) for the treatment of benign prostatic hyperplasia (BPH). Methods: A systematic literature review was conducted using major databases including Ovid-MEDLINE, Ovid-Embase, Cochrane Library, KoreaMed, KMBASE, and RISS. The final search date was July 18, 2023. Only randomized controlled trials (RCTs) comparing PAE and TURP were included. Two independent reviewers performed study selection, data extrac- tion, and risk of bias assessment. Safety was evaluated based on adverse events and complications, while efficacy was assessed through the International Prostate Symptom Score (IPSS), quality of life, urodynamic parameters (maximum urinary flow rate), prostate volume, patient satisfaction or sur- gical success rate, and operative time. Meta-analysis was conducted using RevMan 5.3. Results: A total of four RCTs were included. In terms of safety, there were no significant differences in the inci- dence of major complications such as bleeding, infection, and urinary retention between the two procedures. PAE tended to have a lower incidence of sexual dysfunction compared to TURP. Fluo- roscopy-related radiation exposure and duration were reported to be within acceptable ranges in some studies, and no radiation-related adverse events were identified. Regarding efficacy, PAE showed com- parable improvements in IPSS and quality of life to TURP, but demonstrated inferior outcomes in urodynamic parameters, prostate volume reduction, and surgical success rate. However, patient sat- isfaction was significantly higher in the PAE group (p<0.001). Conclusion: PAE was associated with a lower incidence of major complications and acceptable levels of radiation exposure. It also tended to have less impact on sexual function compared to TURP. Although PAE showed inferior outcomes in certain efficacy measures such as urodynamic parameters, patient satisfaction was higher. Therefore, PAE may be considered a safe and effective alternative treatment, particularly for patients unsuitable for surgery or for whom preservation of sexual function is a priority.
