초록

The Pharmacy of Eulji University Hospital has consulted central PN since November, 2007. The purpose of our study was to evaluate appropriate initial total calories, initial amount of protein and duration of central PN administration in adult patients because they already received PN before consulting. We assigned adult patients who were in hospital and received central PN from April, 2008 to July, 2008 into two groups-ICU group(n=13) and general ward group(n=26). We retrospectively analyzed initial total calories, initial amount of protein, target total calories, target amount of protein and duration. The initial total calories of ICU group were 30.21±7.64kcal/kg/day; target amount of protein 1.19±0.13g/kg/day. Also, the initial total calories of general ward group were 35.59±6.21kcal/kg/day; initial amount of protein, 1.2±0.38g/kg/day; target total calories, 32.013.53kcal/kg/day; target amount of protein, 1.14±0.15g/kg/day. The rate of ICU group who received adequate initial total calories and initial amount of protein was 7.69%(1/13) and the rate of general ward group was 19.23%(5/26). We decided that it was adequate to use PN or kept NPO for 7days and over was 46.15%(6/13) while the rate of general ward group was 76.92%(20/26). It is desirable that patients start to receive PN after consulting from hospital pharmacy. However, we consider that it is impossible to response all the consultation requests immediately. We suggest the doctors administer 30kcal/kg/day as total coaories, 1.2g/kg/day as protein to patients in ICU and 32kcal/kg/day as total calories, 1.15g/kg/day as protein to patients in general ward in case the patients receive central PN before consulting from hospital pharmacy
