초록

Background: The etiology of chronic idiopathic urticaria(CIU) is not completely clear, and there are a few antibodiesincluding anti-FcεRI antibody, anti-immunoglobulin E (IgE)antibody, anti-Helicobacter pylori (HP) antibody, and antithyroglobulinantibody (TGAb) that correlate with CIU. Objective: To investigate the correlation between anti-mastcell antibodies and CIU and to determine which of theseantibodies plays the most important role in CIU pathogenesis. Methods: A total of 100 patients with CIU, acuteurticaria (AU) patients, and normal controls were included. The autologous serum skin test (ASST) was performed, andallergen was detected by enzyme-linked immunosorbentassay (ELISA) in each subject. Serum levels of IgE, anti-FcεRIantibody, anti-IgE antibody, anti-HP antibody and TGAbwere measured by ELISA to determine the relevance of theseantibodies with CIU. Results: 1. Food or inhalant allergenswere detected in most AU patients (86%), but no allergenwas detected in CIU patients or in normal controls. 2. Serumanti-FcεRI antibody level was significantly higher in the CIUgroup than that in the AU group (p< 0.05) and normal controls(p<0.05). Serum anti-IgE antibody level was also higher inthe CIU group than that in the AU group (p<0.05) andnormal controls (p<0.05). 3. IgE level was lower in the CIUgroup than that in the normal controls (T=190.00, p<0.05),but IgE level increased in the AU group compared with thatin the normal controls (T=226.00, p<0.05). 4. The ASSTpositive rates in the CIU and AU groups were 53.4% and12.6% respectively, and ASST was negative in all normalcontrols. The anti-FcεRI antibody level was higher inASST-positive CIU patients than that in ASST-negative CIUpatients (T=101.73, p<0.05), whereas no anti-IgE antibodydifference was observed between these two groups(T=312.04, p>0.05). 5. In anti-HP antibody positive andTGAb positive CIU patients, anti-FcεRI antibody positiverate was higher than AU patients (p<0.01) and normalcontrols (p<0.01), but no difference were observed inanti-IgE antibody (p>0.05). Conclusion: CIU is notidiopathic, as most CIU could be defined as autoimmuneurticaria. The anti-FcεRI and anti- IgE antibodies play a keyrole in CIU pathogenesis. Contrary to some previous reports,anti-HP antibody and TGAb have an indirect but not a directcorrelation with CIU.
