Background: Balneotherapy, although not a well-establisheddermatological treatment, is thought to have therapeuticproperties for psoriasis and is used as an alternative treatmentmodality throughout the world. Objective: To evaluate themechanism underlying the therapeutic immunologic effectsof thermomineral water. Methods: A murine model ofimiquimod-induced psoriasis-like skin inflammation wasused for evaluating the therapeutic effects of balneotherapywith Hae-Un-Dae hot spring mineral water. The clinicalimprovements were evaluated by a dermatologist. Lesionalcytokines, including interleukin (IL)-17A, IL-23, and IL-22,were quantitatively measured by real-time reverse transcriptasepolymerase chain reaction. Serum levels of interferon-γ, IL-4, IL-5, and IL-17A were measured byenzyme- linked immunosorbent assay. T cell proportions inthe spleen were evaluated by flow cytometry, and histopathologicalevaluation of the skin was also performed. Results: The mineral water balneotherapy group showedfaster improvement in skin erythema and scales than thedistilled water bathing group. A substantial reduction wasobserved in the lesional mRNA levels of IL-17A and IL-23 inthe mineral water group. Serum levels of IL-4 and IL-5 weresignificantly decreased in the mineral water group but not inthe distilled water group. Normalized T-cell proportionswere observed after bathing. Conclusion: Balneotherapyshowed immunomodulatory effects in a psoriasis-likemurine model. Balneotherapy suppressed lesional IL-23 andIL-17A, which are important cytokines in the pathogenesis ofpsoriasis. These results suggest that balneotherapy can beused as an effective and safe treatment for psoriasis.
