비교적 흔한 이차성 고혈압: 원발성 알도스테론증의 역학 및 진단과 치료

초록

원발성 알도스테론증은 이제까지 알려져 왔던 것과 달리 비교적 흔한 이차성 고혈압이다. 최근 시행된 역학연구에 의하면 원발성 알도스테론증은 대체로 전체 고혈압 환자의 10%를 차지하는 것으로 알려져 있다. 원발성 알도스테론증에서 나타나는 고알도스테론혈증은 심장섬유화를 일으키며 뇌졸중과 신장질환을 일으키는 원인이 되므로 혈압 조절뿐만 아니라 알도스테론의 작용을 차단할 수 있는 알도스테론 수용체 길항제의 투여가 반드시 필요하다. 선종에 의한 원발성 알도스테론증은 복강내시경으로 부신을 절제하면 대부분의 경우에서 고혈압의 완치나 호전을 기대할 수 있다. 선종이 아닌 경우에도 알도스테론 수용체 길항제인 spironolactone를 투여하면 저칼륨혈증을 효과적으로 교정하고 고혈압을 좀 더 잘 조절할 수 있다. 원발성 알도스테론증의 진단은 알도스테론/레닌 비율을 이용하여 쉽게 선별할 수 있고 일차 진료 현장에서도 수월하게 확진할 수 있으며, 치료할 경우 고혈압의 완치를 기대할 수 있으므로 적극적인 선별이 필요할 것으로 생각한다.
