담석증 치료의 최신지견

Overview of Treatment of Gallstone Diseases

생활 양식이 점차 서구화되어감에 따라 우리나라 담석증 인구는 급격히 증가하여 유병률이 2～3%에 달하고 인구의 5～15%는 나이가 들면서 담석을 갖게 될 것으로 추정한다. 1980년대 이전만 해도 서양과 달리 총담관 내나 간내담관 내에 발생하는 색소성담석이 대부분이었으나 서구화의 영향으로 담낭담석의 비율이 85% 이상으로 증가하고 있다. 그러나 아직도 총담관담석이나 간내담관담석도 10% 이상을 차지하여, 발생부위에 따른 담석증의 임상적 특징, 치료법에 대한 관심이 필요하다. 특히 최근 다양한 담석치료법이 급속도로 개발되고 있어 환자의 임상적 상태, 경제적 요건 및 선호도 등을 총괄적으로 고려하여 개개인에게 가장 적절한 치료법을 결정하여야 하며 이를 위해서는 각종 치료법의 장단점 및 적응증을 숙지하고 있어야 한다. 담석의 제거는 과거처럼 외과의사의 몫만도 아니고 또 진보된 내시경적 기술을 맹신하여 내과적으로만 치료할 것을 고집할 것도 아니라 외과, 내과 및 방사선과 의사가 팀을 형성하여 논의하고 연구하여 가장 적절한 치료법을 찾는 데 힘써야 할 때이다.
