= Abstract = Objectives: This case report aims to 1) propose innovative strategies to strengthen Gyeongsangbuk-do’s regional healthcare system through collaborative governance; 2) assess the effectiveness of public–private partnerships and suggest metropolitan and provincial policy measures to sustain essential health services; and 3) underscore the crucial role of policy leadership in establishing governance frameworks that enhance regional self-sufficiency in healthcare delivery. Methods: This study analyzed Gyeongsangbuk-do’s challenging policy cases through the researcher’s participatory observation of policy processes, together with a review of working documents and policy research reports from the Gyeongsangbuk-do Division of Public Medicine and Public Health Policy Institute, as well as a literature review on collaborative governance. Results: 1) Collaborative governance: A public-private framework strengthened essential healthcare delivery and regional self-sufficiency through joint policy development and crisis coordination. 2) Workforce expansion: Targeted programs addressed gaps in rural/public hospitals via recruitment incentives and retention strategies. 3) Clinical training & quality programs: Mentorship systems and standardized protocols improved care quality through public-private collaboration. 4) Regional self-sufficiency in healthcare delivery: ① One-Hour Patient Care System: Guaranteed critical care access within one hour. ② Severe Emergency Transfer Policy: Streamlined inter-facility coordination, contributing to reduced mortality. Conclusions: This study demonstrates that sustained public-private collaborative governance is feasible across regions between Gyeongsangbuk-do and Daegu Metropolitan City. Gyeongsangbuk-do’s one-hundred percent financial commitment and policy leadership underscore the importance of governance in building, maintaining, and strengthening the local health care system. This model provides a replicable blueprint for balancing equity, accessibility, and quality in healthcare delivery, emphasizing the transformative potential of provincial-level leadership.
