담즙정체간염의 형태를 보인 전염단핵구증 1예

Epstein-Barr virus (EBV)는 herpesviridae family의 일원으로서 숙주의 면역상태에 따라 여러가지 임상상을 보이며, 다양한 질환과 연관이 있다.1 만성 활동 EBV 감염에서 혈구탐식증후군(hemophagocytic syndrome), 과립림프구증식질환(granular lymphocyte proliferative disorder) 및 비호지킨림프종과 같은 림프증식 질환과의 관련성 있으며,2 T세포 림프종과 호지킨림프종 및 비인두암종에서 EBV가 발견된다.3,4 중증감염의 예로는 바이러스연관 혈구탐식증후군이 동반된 간을 침범한 중증 만성 활동 EBV 감염이 있다.5 그 외에도 EBV는 후천면역결핍증 환자에서 구강모백색판증(oral hairy leukoplakia)과 연관이 있는 것으로 보고되었으며6 건강한 청소년 및 성인에서 Heterophile 양성 전염단핵구증의 원인 인자로 잘 알려져 있다.7 EBV 감염은 흔히 간을 침범하나 대개 경하여 임상적으로 뚜렷하지 않고 저절로 회복된다.8 일반적으로 바이러스간염은 매우 다양한 임상 양상과 신체 소견 및 검사 소견을 보여 혈청 및 조직에서 바이러스 표지자를 확인하기 전까지는 그 원인을 감별하기가 쉽지 않다. 많은 바이러스가 급성 간염의 원인이 될 수 있으나 EBV가 성인에서 기술된 예는 흔치 않으며 특히 담즙정체 양상을 보이는 경우는 드물다.9 저자 등은 간기능검사에서 담즙정체 양상을 보이면서, 혈청검사 및 간조직검사로 EBV 간염으로 진단한 1예를 경험하였다.
Abstract
Acute viral hepatitis in human can be caused by a large number of viruses with a wide range of clinical manifestations and laboratory findings. EBV is a rare causative agent of an acute hepatitis, during the course of infectious mononucleosis. Hepatic manifestations of EBV are usually mild and resolve without serious complications. EBV is rather uncommonly confirmed as an etiologic agent in acute viral hepatitis of adults and it rarely causes cholestatic hepatitis. We report a case of EBV hepatitis with cholestatic feature that was verified through serum viral marker and liver biopsy.
