장루미보유 직장암 환자의 불안과 우울 및 관련요인

Anxiety, Depression and their Correlates in Rectal Cancer Patients Without Ileostomy

초록

목적: 본 연구의 목적은 직장암 환자의 수술 후 배변기능, 건강통제위, 불안과 우울 간의 관계를 파악하는 것이다. 연구방법: 본 연구는 상관관계 연구이다. 대상자는 대장암센터 외래를 방문하는 직장암 환자 200명이다. 연구도구는 배변기능, 건강통제위, 불안과 우울을 측정하기 위한 도구이다. 수집된 자료는 기술통계. t-test, one-way ANOVA, Pearson’s correlation으로 분석하였다. 결과: 배변기능은 평균 34.44±5.73점이었다. 내적건강통제위, 우연건강통제위, 의사건강통제위와 타인건강통제위의 평균은 각각 29.06±6.81, 22.41±6.96, 15.88±2.85, 10.72±4.68점이었다. 불안과 우울은 각각 평균 4.49±3.95, 6.25±3.98점이었다. 배변기능은 불안(r=-.180,p=.011),우울(r=-.267,p<.001)간에 통계적으로 유의한 음의 상관관계가 나타났고, 내적건강통제위와 우울(r=-.149,p=.035)간에도 통계적으로 유의한 음의 상관관계가 나타났다. 우연건강통제위와 우울(r=.146,p=.039)간에 통계적으로 유의한 양의 상관관계가 나타났고, 불안과 우울(r=.651,p<.001)간에도 통계적으로 유의한 양의 상관관계가 나타났다. 결론: 직장암 수술 후 배변기능이 낮은 환자의 불안과 우울을 사정하고, 내적건강통제위를 강화할 수 있는 간호중재 개발과 이에 대한 교육이 필요하다고 사료된다.

Abstract

Purpose: This study aimed to identify the relationships among bowel function, health locus of control, anxiety, and depression in patient with rectal cancer. Methods: This study utilized a descriptive correlational design. Subjects were 200 rectal cancer patients who attended out-patient clinic in a colorectal cancer center. The study instruments included the scales for bowel function, health locus of control, anxiety, and depression. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, and Pearson's correlation coefficient. Results: The mean score of bowel function was 34.44±5.73. The mean scores of internal HLOC, chance HLOC, doctor HLOC, and powerful others HLOC were 29.06±6.81, 22.41±6.96, 15.88±2.85, and 10.72± 4.68 respectively. The mean scores of anxiety and depression were 4.49±3.95 and 6.25±3.98, respectively. There were significant negative correlations between bowel function and anxiety (r=-.180, p=.011); between bowel function and depression (r=-.267, p<.001); and between internal health locus of control and depression (r=-.149, p=.035). There were significant positive correlations between chance health locus of control and depression (r=.146, p=.039), and between anxiety and depression (r=.651, p<.001). Conclusion: It is suggested that anxiety and depression for the patients with low bowel function after colorectal cancer surgery should be evaluated and nursing interventions to enhance internal health locus of control need to be developed.
