남성 척수손상 장애인의 성실태 및 성적응에 미치는 요인

초록

성재활(sexual rehabilitation) 프로그램 개발을 위해 남성 척수손상 장애인의 성실태(sexual activity) 및 성적응(sexual adjustment)에 미치는 변인을 확인하고자 실시하였다. Methods : 성인 남성 척수손상 장애인 135명을 대상으로 2014년 3월～ 7월 까지 구조화된 설문지로 조사한 서술적 단면연구이다. Results : 척수 손상이후 성적응은 낮은 수준을 유지하고 있으며, 성생활의 가장 큰 장애요소는 ‘발기기능 저하'(65.2%) 였다. 성욕구(Sexual desire)는 87.4%에서 회복되었고, 1회 이상 성교(intercourse)를 경험한 경우는 62.4%였다. 발기기능(erection function)에 문제가 없는 경우는(complete) 13.0%였고, 감소(incomplet) 62.6%, 불가능(no) 24.4%였다. 사정기능(Ejaculation)에 문제가 없는 경우(complete)는 8.1%였다. 발기기능은 불완전 척수 손상(incomplete SCI)에서 더 좋았고(p<.001), 상부운동신경원손상은 하부운동신경원 손상보다 더 좋았다(p=016). 성적응(Sexual Adjustment)과 관련된 요인은 나이, 직업, 손상정도, 손상기간, 발기기능, 성건강이었다. 성적응(sexual adjustment)은 성건강(MSHQ) 변인으로 24.2%를 설명하였다. Conclusion : 남성 척수손상 장애인의 가장 큰 성생활 불만족 원인은 발기기능 저하였다. 발기와 사정은 손상 레벨과 손상 정도에 따라 차이가 있었다. 성적응에 영향을 주는 주요인은 성건강이었다.

Abstract

Purpose: The purpose of this study was to identify sexual activities of males with spinal cord injury (SCI) andinfluential factors of sexual adjustment. Methods: A correlational survey was conducted among a total of 135males with SCI. Results: After SCI, patients maintained low levels of sexual adjustment, and a main obstacleof sexual rehabilitation was decreased erectile function (65.2%). Sexual desires recovered in 84.4%, with62.4% patients reporting more than once intercourse experiences. The erectile function of participants withnormal, decreased or no erectile function were 13.0%, 62.6% and 24.4%, respectively. 8.1% of the participantsreported no problem with ejaculation. Patients with incomplete SCI had better preserved erectile function thanthose with complete SCI (t=-4.627, p<.001). Patients with upper motor neuron injury had better preservederectile function than those with lower motor neuron injury (t=2.446, p =.016). Sexual adjustment wasrelevant to age, job, degree of injury, post-injury period, sexual desire, intercourse experience, erectiontherapy, erectile function, and sexual health. Sexual adjustment was a factor of sexual health with a power of24.2%. Conclusion: The main obstacle of sexual rehabilitation for males with SCI decreased erectile function. Erection and ejaculation are dependent on the severity and level of SCI. The major influence on sexualadjustment is sexual health.
