위장관 암환자의 외상 후 성장 영향요인

Factors Influencing Post-Traumatic Growth in Patients with Gastrointestinal Cancer

초록

목적: 본 연구는 위장관 암환자의 외상 후 성장에 영향을 미치는 요인을 조사하기 위해 시도되었다. 방법: 본 연구는 경기도 소재 A 대학병원의 위암 센터와 대장암 센터에서 위장관 암으로 진단받고 치료 후 외래 추적 중인 대상자 120명을 대상으로 하였다. 자료 수집은 인구사회학적 특성, 질병관련 특성, 외상 후 성장, 지각된 질병 영향, 낙관성, 우울, 사회적 지지로 구성된 자기 보고식 설문지를 이용하였다. 수집된 자료는 독립표본 t-검정, 일원배치 분산분석, 상관분석과 위계적 다중회귀분석으로 분석하였다. 결과: 위장관 암환자의 외상 후 성장 평균은 52.74점(총점 92점)이었다. 외상 후 성장 하부 영역에서 삶의 소중함 수준 영역이 가장 높았고, 대인관계 영역이 가장 낮았다. 외상 후 성장과 관련된 요인은 낙관성(β=.36, p<.001), 사회적 지지(β=.31, p<.001), 우울(β=-.27, p<.001)이었다. 본 연구 회귀모형의 외상 후 성장에 대한 설명력은 45.1% 였다(F=13.20, p<.001). 결론: 국내 위장관암 유병률이 증가하고 있는 점을 고려할 때 이들의 외상 후 성장에 영향을 미치는 일반적, 심리적, 사회적 요인을 규명하는 것은 간호실무 향상에 중요하다. 위장관 암환자의 외상 후 성장을 촉진하기 위해 낙관성과 사회적 지지를 증진시키고, 우울 수준을 낮추는 프로그램 개발, 적용한다면 위장관 암 환자의 외상 후 성장 향상에 기여할 수 있을 것이다.

Abstract

Purpose: The purpose of this study was to investigate the influencing factors of post-traumatic growth in patientswith gastrointestinal cancer. Methods: A cross-sectional study was conducted, enrolling 120 patients withgastrointestinal cancer. Their general characteristics, disease-related characteristics, perceived illness intru-siveness, levels of optimism, social support, and post-traumatic growth were assessed through self-admin-istered questionnaires. The collected data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation, and multiple regression analysis. Results: The mean score of post-traumatic growth was 52.74 points(total score of 92 points). Among the subscales of post-traumatic growth, levels of the preciousness of life were the highest, and relating with others was the lowest. The post-traumatic growth wasfound to be significantly correlated with optimism (r=.48, p<.001), social support (r=.47, p<.001), and depression (r=-.37 p<.001). Factors associated with post-traumatic growth were optimism (β=.36, p<.001), social support (β=.31, p<.001), and depression (β=-.27, p<.001). Conclusion: Given that prevalence ofgastrointestinal cancer is increasing in Korea, identifying general, psychological, and social factors affecting post-traumatic growth among this population will be helpful in clinical practice. Integrated strategies to increase optimism and social support and lower depression should be considered to improve the post-traumatic growth of patients with gastrointestinal cancer.
