Cerebrovascular reactivity (CVR) refers to the capacity of cerebral blood vessels to regulate local blood flow by adjusting their diameter in response to physiological stimuli. Recently, new CVR–magnetic resonance imaging (MRI) techniques that do not require physiological stimuli have been introduced. This review provides a comprehensive analysis of CVR measurement methods, including resting-state functional MRI and task-based approaches. The physiological mechanisms underlying CVR are discussed, along with the strengths, limitations, and clinical applications of these measurement methods. Advancements in analytical techniques and alternative approaches are essential to enhance the reliability of CVR estimation and its integration into clinical practice.
