Objective: This study aimed to systematically review and analyze the effects of herbal medicine treatment on chronic fatigue syndrome (CFS) after COVID-19. Methods: CFS-related studies published from January 1, 2020 to August 31, 2024 were screened from databases. A total of 15 randomized controlled trials (RCTs) were selected. A meta-analysis was conducted based on outcome measurements, such as the total effective rate (TER). Cochrane’s risk of bias 2 (RoB 2) was used for the qualitative evaluation of the papers. Results: All the studies were conducted in Asia, with 80% from China. The CFS diagnoses were mostly based on the Centers for Disease Control and Prevention (CDC) 1994 guidelines. The most common TKM diagnosis after COVID-19 was spleen deficiency, and the most frequently used outcome measurement was the TER. A meta-analysis showed statistically significant improvements of herbal treatments compared to Western medicine and conventional herbal medicine treatment in most studies. Conclusion: After COVID-19, herbal medicine treatment for CFS has focused on spleen deficiency syndrome, considering viral infection-induced immune system dysfunction as the primary cause. Newly researched herbal medicines have shown significant therapeutic effects. However, as this study has some limitations, improvements in the quality of research are required.
