경막외강 내 Meperidine 지속 주입에 의한 술 후 통증관리

본 실험에서는 경막외 meperidine을 단독으로 투여하였을 때 bupivacaine을 혼합하여 투여한 경우와 비교하여 수술 후 2일째 방문시점의 통증의 정도가 아닌 수술 후 경험한 통증의 종합적인 평가에 따른 VAS와 추가진통제 사용의 빈도와 총량에서 통계적으로 유의한 차이를 보이지 않았고 부작용의 빈도에서도 양 군 간에 차이가 없었다.결론적으로 상복부 수술 환자의 술 후 통증완화를 위하여 흉부 경막외강으로 3.5 mg/ml (65세 이상은 3 mg/ml) meperidine을 체중 60 kg을 기준으로 체중의 증감에 따른 용량을 고려한 후 2 ml/hr로 주입하는 것은 효과적이었다. 따라서 0.15% bupivacaine과 혼합하여 사용한 경우와 비교하여 국소마취제의 혼합 없이 경막외로 meperidine을 단독으로 투여하여도 부작용의 증가 없이 효과적으로 통증 조절을 할 수 있으리라 생각된다.
