Primary Nasopharyngeal Tuberculosis without Cervical Lymphadenopathy : Report of a Case and Review of the Literature

비인두 결핵은 드문 질환이며 다른 부위 결핵이 없으면서 비인두 결핵이 단독으로 발병하는 경우는 더욱 드문 실정이다. 18세 남자 환자가 수 년간의 좌측 비폐쇄 증상을 호소하면서 내원하였는데 재채기, 코의 가려움증 등은 있었으나 기침, 객담 및 발열 등의 전신 증상은 없었다. 과거력상 폐결핵의 병력은 없었다. 이학적 검사상 경부 림프절병증의 소견은 없었고 비강 내시경 검사상 비중격은 좌측으로 편향되어 있었고 점막으로 둘러싸인 2.0×1.5 cm 크기의 독립된 종괴가 비인두의 중간선 부위에서 관찰되었다. 경부 컴퓨터 촬영 검사에서도 1.7 cm 크기의 종괴가 관찰되었으나 경부에서 비정상적인 림프절 비대 등의 소견은 없었다. 비인두 종괴의 조직병리학 검사에서는 다핵 거대세포와 건락육아종의 소견이 관찰되어 원발성 비인두 결핵으로 진단하였으며 항결핵제를 투여하였다. 저자들은 경부 림프절병증을 동반하지 않은 원발성 비인두 결핵 환자 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
Abstract
Nasopharyngeal tuberculosis is an uncommon disease, and isolated nasopharyngeal tuberculosis is an even rarer condition. An 18-year-old male came to our clinic with left nasal obstruction. Upon physical examination, there was no cervical lymphadenopathy. Nasal endoscopic examination revealed an isolated 2.0×1.5 cm sized pedunculated mass at the midline of the nasopharynx. Histopathological findings of the nasopharyngeal mass showed a caseating granuloma with multinucleated giant cells. He was diagnosed with nasopharyngeal tuberculosis and treated with antituberculous agents. We herein report a case of isolated nasopharyngeal tuberculosis without cervical lymphadenopathy with review of the literature.
