양로시설 거주 노인의 요실금 및 하부요로증상

A Study of Urinary Incontinence and Lower Urinary Tract Symptoms for the Residents of Nursing Homes

-연구배경 본 연구는 장기양로시설 노인의 요실금 및 하부요로증상의 실태를 파악하여 효과적인 배뇨관리 프로그램 개발을 위한 기 초자료로 제공되고자 실시되었다 .방 법 2003년 5월부터 10월 사이에 J도 3곳의 양로시설과 S시 두 곳의 양로시설에 거주하고 있는 남녀 노인 112명을 대상으로 설문지를 이용하여 요실금 및 하부요로증상에 대한 조사를 실시하였다 . 대상자의 일반적 특성, 일상생활활동정도, 인지상태, 요실금과 하부요로증상, 그리고 지각된 배뇨증상정도는 빈도와 백분율을 산출하였고 , 연령 및 성별에 따른 요실금 및 하부요로증상은 t-test, ANOVA를 시행하였다. 결 과 요실금은 71명으로 전체조사 대상자의 63.4%인 것으로 나타났다 . 연령에 따른 요실금 유병 상태는 80대이상군에서 68.1%로 가장 높았고, 70대가 61.0%, 60대가 40.2%의 순이었으며, 남성의 경우 39.0%, 여성은 68.0%에서 요실금을 가지고 있었다 . 요실금 유형은 혼합성요실금 40.8%, 복압성요실금 31.0%, 절박성요실금 28.2%의 순을 나타내었다 . 하부요로증상의 평균점수는 12.2±8.6점으로, 경증군 33.9%, 중등증군 42.0%, 중증군 24.1%이었다. 연령과 성별에 따른 하부요로증상점수는 유의한 차이가 없는 것으로 나타났다 . 대상자가 지각하는 배뇨증상의 심각도는 보통이다가 35.7%, 심각하다가 12.5%로 응답하였으며, 전체 대상자중 최근 일년동안 요실금 및 하부요로증상으로 의사의 진찰과 치료를 받은 경험자는 16.1%로 나타났다.

Abstract

Background This study was designed to provide the basic data for the development of the effective programs for voiding management by identifying urinary incontinence and lower urinary tract symptoms among the residents in nursing homes. Methods The study was performed for urinary incontinence and lower urinary tract symptoms with using questionnaires from May to October, 2003. The subjects consisted of 112 residents in 3 nursing homes located at J city and 2 nursing homes located at S city. The data were analyzed by frequency, t-test, and ANOVA with using SPSS program. Results According to the investigation and analysis, 63.4% of the subjects, which was 71 subjects, had urinary incontinence. As for the prevalence of urinary incontinence by age, the eighties was highest 68.1%, the seventies 61.0%, the sixties 40.2%. The prevalence of urinary incontinence for the male was 39.0%, for the female 68.0%. In the type of urinary incontinence, mixed UI was 40.8%, stress UI 31.0%, urge UI 28.2%. Mean score for lower urinary tract symptoms was 12.2±8.6 and the group for mild symptoms was 33.9%, the group for medium symptoms 42%, the group for severe symptoms 24.1%. There was no significant difference in the score for lower urinary tract symptoms by age and sex. In the perceived severity of voiding symptoms, 12.5% of the respondents perceived severely and 35.7% of the respondents perceived moderately. 16.1% of the respondents had doctor's diagnosis and treatment for urinary incontinence and lower urinary tract symptoms during the past one year.
