자가통증조절장치를 이용한 통증관리 교육이 수술환자의 통증에 미치는 효과

초록

연구배경: 본 연구는 수술 전 제공한 자가통증조절 장치를 이용한 통증교육이 수술환자의 통증조절 만족도에 미치는 효과를 분석함으로써 수술 후 환자의 통증조절에 대한 간호를 제공하고자 시도된 비동등성 대조군 전․후 시차 설계를 이용한 유사 실험연구이다. 방법: 연구대상은 2007년 4월 9일부터 8월 24일까지 서울시내에 소재하는 P의료원에서 산부인과 수술(전자궁적출술)후 정맥 자가통증조절 장치를 사용하는 환자 중 선정 기준에 부합한 실험군 28명, 대조군 28명 총 56명으로 하였다. 수집된 자료는 SPSS 프로그램으로 동질성 검증은 χ²-test와 t-test로, 가설 검증은 paired t-test와 t-test로 분석하였다. 결과: ‘수술 전 통증관리 교육을 받은 실험군은 통증관리 교육을 받지 않은 대조군보다 수술 후 자가통증조절장치 사용에 대한 지식정도가 높을 것이다’라는 가설 1은 기각되었다. ‘수술 전 통증관리 교육을 받은 실험군은 통증관리 교육을 받지 않은 대조군보다 수술 후 통증약물 사용에 대한 태도가 긍정적일 것이다’라는 가설 2는 지지되었다. ‘수술 전 통증관리 교육을 받은 실험군은 통증관리 교육을 받지 않은 대조군보다 수술 후 통증정도가 낮을것이다’라는 가설 3은 기각되었다. ‘수술 전 통증관리 교육을 받은 실험군은 통증관리 교육을 받지 않은 대조군보다수술 후 통증조절 만족도가 높을 것이다’라는 가설 4는 지지되었다. 결론: 수술 전 체계적이고 개별화된 자가통증조절장치를 이용한 통증관리 교육은 대상자의 수술 후 통증약물에 대한 태도를 긍정적으로 변화시켜 수술 후 통증정도를 감소시키고 통증조절 만족도를 높이는 데 효과적인 간호중 재임을 알 수 있었다.

Abstract

Background: The purpose of this study was to evaluate pain management in postoperative patients by assessing the effectiveness of preoperative patient-controlled analgesia (PCA) education on postoperative pain. Methods: Our subjects included 56 patients admitted for gynecologic surgery at a university hospital setting. They were divided into experimental (n=28) and control (n=28) groups. Knowledge of PCA use, attitude toward the use of analgesic drugs, severity of pain, and pain control satisfaction were investigated through questionnaires completed before the pain management education and during the postoperative period. Results: The knowledge of PCA use was better in the experimental group, but it was statistically not significant. Pain was more severe in the experimental group, but it was also statistically not significant. The attitude toward the use of analgesic drugs was significantly more positive, and the level of atisfaction of pain control was significantly higher in the experimental group. Conclusions: Preoperative education using PCA could help patients have a positive attitude toward the use of analgesic drugs and promote better satisfaction of pain control.
