목적 : 본 연구는 파킨슨병 환자의 지역사회 중심 중재를 체계적으로 고찰하고, 기존 보고된 중재 방법과그 효과성을 정리 및 분석하는 데 중점을 두었다. 본 연구 결과는 추후 임상 현장에서 활용 가능한 가이드라인을 마련하는 데 기초 자료로서 의의를 둔다. 연구방법 : PRISMA 가이드라인과 P.I.C.O를 사용하여 수행하였다. 데이터베이스는 PubMed, Embase, Web of Science, ScienceON을 사용하여 2025년 5월까지 출판된 문헌을 검색하였다. 영어로 작성되고 전문이 공개된 문헌만을 포함하였다. 검색어는 파킨슨, 지역사회, 재활 등 세 가지로 세분화하여 검색하였다. 총 8,367편 중 중복제거 후, 연구자가 개별적으로 제목, 초록, 전문을 검토하여 선정 및 배제기준에 따라 최종 20편이 도출되었다. 결과 : 최종적으로 선정된 20편의 무작위 대조군 연구는 아시아, 유럽, 북아메리카, 오세아니아 등 9개국에서 수행되었으며, 전체 연구 중 80%(16편)는 균형, 보행, 유산소 운동 등 운동기능 향상을 목적으로 한중재였다. 나머지 20%(4편)는 우울, 피로, 수면, 인지기능 등 비운동성 증상을 대상으로 하였다. 결론 : 본 연구 결과는 약물 치료 중심에서 벗어나 운동성과 비운동성을 포괄하는 다학제적 지역사회 중재전략의 필요성을 나타내며, 향후 다양한 연구 설계와 장기적 효과 검증이 요구된다.
Abstract
Objective : This study conducted a systematic review of community-based interventions for individuals with Parkinson's disease, analyzing intervention methods and their effectiveness. The findings will provide foundational data for developing practical guidelines for future clinical settings. Methods : The study followed the PRISMA guidelines and used the PICO framework. A literature search was conducted using the PubMed, Embase, Web of Science, and ScienceON databases through May 2025. Inclusion criteria were limited to English language articles with full-text availability. Search terms were categorized into three groups: Parkinson's, community, and rehabilitation. From 8,367 initial articles, duplicates were removed, and researchers reviewed titles, abstracts, and full texts. Using predefined criteria, 20 articles were selected. Results : The 20 randomized controlled trials were conducted across nine countries in Asia, Europe, North America, and Oceania. Of these, 80% (16 articles) focused on motor functions, including balance, gait, and aerobic exercise. The remaining 20% (4 articles) addressed non-motor symptoms such as depression, fatigue, sleep, and cognitive function. Conclusion : These results highlight the need for multidisciplinary community-based intervention strategies that address both motor and non-motor dimensions beyond medication-focused treatments. Future research should employ diverse methodological designs and assess long-term intervention effects.
