유전치 심미수복치료를 위한 임상적 고려

소아치과 임상에서 느껴지는 유전치 색조의 특성은 영구전치의 것과는 다르다. 유치의 색조는 영구치에 비해 더 밝고 청색을 띤다고 알려져 있으며 colorimeter를 이용한 과학적 측정값에서도 이와 같은 경향을 보여주고 있다. 이에 따라 영구전치의 색조체계에 기초한 기존의 심미수복재료의 색조를 그대로 유전치에 적용했을 경우, 임상가는 종종 색조의 부조화를 경험하게 되며, 심지어는 유치용으로 제작된 pontic에서도 이러한 부조화가 관찰되고 있다. 필자는 만 2세부터 5세까지의 남녀 어린이 100명을 대상으로 건전한 상악 좌우 유중절치 및 유측절치를 대상으로 colorimeter를 이용하여 각 치아의 순면 중앙부의 색조를 측정하여 그 특성을 연구하였다. 남녀 어린이간 명도(CIE L*)의 통계학적 유의차는 없었으나, 증령에 따라 그 값의 감소를 관찰할 수 있었으며, CIE b* 값의 유의성 있는 증가를 알 수 있었다. 하지만, 두 치아간의 색조 차이를 나타내는 ΔE*ab의 값은 동명 치아간, 동일인 내 다른 치아간에 임상적으로 그 차이를 눈으로 구별하기 어려운 정도로 낮은 것으로 보여졌다. 이에 따라 건전한 상악 유전치 색조의 대표값을 구하여 이에 가장 부합하는 색조의 심미수복재료 선정에 이용하기로 하였다.
Abstract
The esthetic demands are growing in treatment of primary maxillary incisors as well as permanent ones. There is something dissatisfying about in the keen eyes of the pediatric dentists on their esthetic results of efforts because current tooth color system is created for the purpose of the application to the permanent incisors. In this study using the scientific color measurement device, I would like to introduce a color scheme for the primary maxillary incisors and suggest clinical tips for the better results of resin composite restorations.
