비스포스포네이트(Bisphosphonate)는 골흡수억제제로서 폐경기 이후 골다공증의 예방 및 치료, 스테로이드에 의한 골다공증의 예방과 치료, 파겟병(Paget’s Disease), 다발성 골수종, 악성종양의 전이예방, 고칼슐혈증에 많이 사용되고 있다. 이 중 골다공증환자에서 발생하는 골절의 예방 및 치료를 위해서 비스포스포네이트는 50%이상 처방되고, 그 수는 점차 증가하고 있다. 2003년 Marx가 미국구강악안면외과학회지에 비스포스포네이트 치료를 받는 36명의 환자에서 무혈성악골괴사(Avascular Necrosis of the Jaw)를 보고한 이후 비스포스포네이트의 부작용으로 그 심각성을 인식하기 시작하였고, 이후 비스포스포네이트 관련 악골괴사(Bisphosphonate related Osteonecrosis of The Jaw;BRONJ)에 대한 보고는 증가하기 시작하였고, 2007년, 2009년 미국구강악안면외과, 미국내분비내과에서도 그 위험성에 대한 Positional Paper를 발간하였고, 2009년 한국에서도 대한내분비학회와 구강악안면외과학회에서 그 위험성을 의사 및 치과의사들에게 환자의 치료에서 주의를 요구하고 있다. 이 논문에서는 골다공증 치료에 많이 사용되고 있는 비스포스포네이트제재와 이에 의해서 발생되는 BRONJ에 대해서 문헌고찰을 통해서 알아보고자 한다.
Abstract
Bisphosphonates are widely used in the management of osteoporosis patients. Many reports and clinical studies have published a relationship between necrotic bone lesions localized to the jaw and the use of chronic bisphosphonate therapy. This osteonecrosis named bisphosphonate-related osteonecrosis of the jaw (BRONJ). This article described the mechanism, chemical structure, indication, risk factor of the bisphosphonate
