세계보건기구 분류에 근거한 악성림프종의 골수침습 빈도 및 양상- 단일 기관 연구

초록

배경 : 지금까지 국내에서 발표된 악성림프종의 골수침습에 관한 연구는 주로 Working Formulation 또는 Revised EuropeanAmerican Classification of Lymphoid Neoplasms (REAL) 분류를 따른 것이었다. 본 연구에서는 WHO 분류에 따라 악성림프종의 아형별 골수침습률과 침습양상을 조사하였다.방법 : 1999년 1월부터 2005년 12월까지 원자력병원에서 악성림프종의 병기 결정을 위하여 골수검사가 의뢰된 악성림프종 환자 507예를 대상으로 하였으며 의무기록, 말초혈액도말 소견, 골수천자도말 소견, 골수생검 소견 및 조직병리 소견을 후향적으로분석하였다. 결과 : 507예의 악성림프종 환자 중 비호지킨림프종은 473예(93.3%)이었고 호지킨림프종은 34예(6.7%)이었다. 비호지킨림프종 전체의 골수침습률은 12.5% (59/473)였고 호지킨림프종은11.8% (4/34)였다. 비호지킨림프종 중에서 B세포림프종은 11.4%(43/377), T세포림프종은 16.7% (16/96)의 골수 침습률을 보였다. B세포림프종 중 많은 아형에서 30% 이상의 골수침습률을 보였으나 광범위대형B세포림프종은 4.6%로 상대적으로 낮았다. 가악성림프종의 골수침습 빈도 및 양상 387장 많이 나타난 침습 양상은 미만형(40.0%)이었으며, 말초혈액침범은 골수침습례의 35.6%에서 나타났다.
