아토피성 피부염 환자에서 혈청 인터루킨-18의 임상적 의의

초록

배경 : 인터루킨(IL)-18은 인터페론 감마(IFN- ) 생산유도인자이지만, 아토피성 피부염을 포함한 Th2 세포 관련 질환에도 기여하는 것으로 보고되어 있다. 본 연구에서는 아토피성 피부염에서 혈청 IL-18의 임상적 의를 알아보고자 하였다.방법 : 아토피성 피부염 환자와 대조군에서 혈청 IL-18, IFN- ,IgE, 혈액 호산구수를 환자의 피부 중증도 점수에 따라 분석하거나 대조군과 비교하였다.결과 : 혈중 IL-18은 아토피성 피부염 환자에서 대조군에 비해유의하게 높았다(332 pg/mL vs 151 pg/mL, P<0.05). 혈중IL-18 (r=0.41, P=0.001), 호산구(r=0.36, P=0.003) IgE (r=0.32, P=0.009) 모두 임상적 중증도와 유의한 양의 상관성을 보였다. 다중회귀 분석결과 또한 IL-18은 IgE와 함께 중증도를 반영하는 독립적 예견자임을 보여주었다(r2=0.25, =0.39, P=0.001및 =0.32, P=0.009).
