기관지 내시경 오염에 기인한 Stenotrophomonas maltophilia의 가유행 발생

배경 : 조선대학교 병원에서 수집된 기관지폐포세척액(bron-choalveolar lavage, BAL) 배양 검체에서 Stenotrophomonasmaltophilia분리빈도의 갑작스런 증가가 인지되었다. 이에 본 연구는 돌발 감염을 의심하여 오염원을 확인하고 이를 제어하기 위해 환경감시 배양을 실시하였다. 방법 :S. maltophilia의 돌발 감염을 의심하여 이를 조사하기위해 기관지 내시경실과 2대의 기관지 내시경에서 1검체를 채취하여 환경감시 배양을 실시하였다. 세 환자의 BAL 검체에서 분리된S. maltophilia 균주들과 환경배양에서 분리된 1균주의 S. mal-tophilia간에 유전적 연관성을 검사하기 위해 pulsed-field gel elec-trophoresis (PFGE)로 유전자 형별 분석을 실시할 때, 이들과 무관한 9균주의S. maltophilia도 대조균주로 함께 검사하였다.결과 : 가유행 이전 6개월 동안에 총 188명 중 5명(2.6%)의BAL 검체에서 S. maltophilia균이 분리된 반면 2006년 3월의 1주일 사이에 BAL 배양이 의뢰된 13명 중 7명(54%)에서 S. mal-tophilia균이 배양되었다. 1개 중 1개의 환경배양 검체에서만 S.maltophilia가 배양되었는데 이는 기관지 내시경 흡인용 도관 내강 세척액에서 분리되었다. 환자에서 분리된 7주와 환경배양에서분리된 1주의 항생제 감수성 결과 양상이 동일하였다. 가유행 균주들을 PFGE로 유전자 형별 분석한 결과 유사한 양상이 관찰된반면 역학적으로 무관한 대조 균주들은 서로 다른 양상을 보였다.
