불안민감성과 자살 사고 및 자살 시도의 관계: 우울의 매개효과

초록

연구목적: 본 연구는 불안민감성 하위 요인(신체적 염려, 인지적 염려, 사회적 염려)과 자살 사고 및 자살 시도의 관계를 살펴보고, 이 관계에서 우울이 매개하는지를 검토하는 데 목적이 있다. 연구방법: 상담 및 심리치료 경험이 있는 성인 123명을 대상으로 불안민감성, 자살 사고, 자살 시도, 우울을 측정하는 설문조사를 실시하였으며, 회귀분석을 포함한 통계분석은 SPSS Macro Process 23.0 program, Process macro (model 4)를 사용하였다. 연구결과: 첫째, 인지적 염려가 자살 위험을 예측하는 주요 요소로 작용하고, 신체적 염려가 자살 사고와 연관이 있는 것으로 나타났으며, 사회적 염려는 낮을수록 자살 사고가 증가하는 경향이 있는 것으로 나타났다. 둘째, 인지적 염려 요인과 자살 사고 및 자살 시도 관계는 우울이 매개하는 것으로 확인되었다. 즉, 인지적 염려가 높아질수록 우울이 증가하고, 증가된 우울은 자살 사고 및 자살 시도로 이어지는 경로가 확인되었다. 결론: 본 연구는 인지적 염려가 자살 사고를 예측하는 독립적인 역할을 할 뿐만 아니라, 우울을 매개로 자살 사고와 자살 시도를 예측하는 중요한 경로를 제시했다. 또한, 신체적 염려는 자살 사고와 연관이 있었고, 사회적 염려가 낮을수록 자살 사고가 증가하는 것으로 확인되었다. 이는 자살 위험 요인을 이해하는데 있어 인지적 요인을 비롯하여 신체적, 사회적 요인을 포함한 다차원적 접근이 필요함을 시사한다.

Abstract

Objectives: This study examines the relationships between sub-factors of anxiety sensitivity (physical, cognitive, and social concerns) and suicidal ideation and suicide attempts, while investigating whether depression mediates these relationships. Methods: A survey assessing anxiety sensitivity, suicidal ideation, suicide attempts, and depression was conducted with 123 adults who had undergone counseling and psychotherapy. Statistical analyses, including regression analysis, were performed using SPSS version 23.0 and the Process macro (model 4). Results: First, cognitive concerns were identified as a major predictor of suicide risk, while physical concerns were associated with suicidal ideation. Social concerns showed an inverse relationship, with lower levels of social concerns correlating with higher levels of suicidal ideation. Second, depression was found to mediate the relationship between cognitive concerns and both suicidal ideation and suicide attempts. Specifically, higher levels of cognitive concerns were linked to increased depression, which in turn led to an increase in suicidal ideation and suicide attempts. Conclusions: This study revealed that cognitive concerns not only play an independent role in predicting suicidal ideation but also serve as a significant pathway to suicidal ideation and suicide attempts through the mediation of depression. Additionally, physical concerns were associated with suicidal ideation, and lower levels of social concerns were linked to an increase in suicidal ideation. These findings suggest the necessity for a multidimensional approach to understanding suicide risk factors, incorporating cognitive, physical, and social dimensions.
