의료기관 기능 재정립을 위한 제도적 개선방안 연구

의료체계와 의료자원의 효율적 운영을 통해 의료서비스를 필요로 하는 국민 모두가 적시에 적정인에 의해 적소에서 적정진료를 이용할 수 있도록 마련된 제도가 의료전달체계이다. 이러한 의료전달체계의 가장 기본은 의료기관 종별로 그 기능과 역할을 구분하여 의료자원의 효율적 활용을 도모하는 것이다. 그러나 현재 우리나라는 의료전달체계가 확립되어 있지 않아 대형병원으로의 환자 쏠림현상이 심각한 상태이며 정부 차원에서도 이러한 문제를 해결하기 위한 여러 가지 정책을 고심하고 있다. 이에 따라 본 연구는 의료기관 종별로 기능을 재정립할 수 있는 제도적 개선방안을 모색하기 위해 의료전달체계, 의료기관 종별 기능과 역할 등을 고찰하고, 의료기관 종별로 그 기능을 재정립할 수 있는 제도적 방안을 살펴보았다. 결과에 따르면 법적으로는 의료법의 의료기관 구분의 규정을 개선하여 각 종별 기능을 구체화하여 제시하고, 국민건강보험법상 요양급여의 절차에 관한 법적 근거를 보다 강화할 필요성이 있다. 제도적으로는 의료기관에 적용할 수 있는 유인책을 통해 의료전달체계의 정립을 꾀할 수 있을 것이다. 이를 위해 의료기관 종별로 적합한 기능을 유인할 수 있도록 차등수가제와 종별가산제의 개선, 진료 의뢰와 회송 활성화를 위한 제도 마련, 병원급 의료기관에 연구기능 확대를 위한 정부의 재정지원 확대를 제안하였다.
Abstract
Health care delivery system has been introduced to provide patients with better health care use and access to the health care providers by efficient operation of limited health care resources. The principle of this system is to practically exploit limited health care resources by differentiation of functional roles among various types of health care providers. However, now the concentration of patients in large hospitals or the distortion of functional differentiation among health care providers has been worse off because of not-cleary-established health care delivery system. Government entities are now also struggling to solve this problem. In this study we analyzed the plans for establishment of health care delivery system by examining the functional role of various health care providers and reviewing. Our findings suggested both a legal and a systematic alternatives to resolve current challenges. First, from the legal approach, medical service act needs to be improved to define the functional role among the different type of health care providers. In addition, a legal background to control referral system also must be reinforced by national health insurance law. Second, judging from the systematical point, it is necessary to employ both incentive and disincentive plans to reform the current inefficient delivery system. For example, out-of-pocket payment for patient who directly visit tertiary hospital without referral should be raised. Also, payment differentiation to providers by volume of patient visits or their size of beds should be reformed. Strategy plan to encourage both patient and providers to have referral motivation should be introduced. Thus, government must financially supports tertiary hospitals on their research and development.
