고관절 전치환술 전후 노인 환자의 인지기능 비교- 여성노인과 남성노인을 대상으로

초록

본 연구는 고관절 전치환술 전후 노인 환자의 인지기능을 성별에 따라 비교하기 위하여 인지능력 측정하는데 가장 널리 사용되고 있고 치매선별검사도구로 이용되고 있는 MMSE-DS를 통하여 측정하였다. 고관절 전치환술을 받은 44명의 환자를 대상으로 수행한 본 연구는 총 3회(수술 전, 수술 일주일 후, 수술 3개월 후)의 MMSE-DS를 반복 측정하여 인지기능의 변화 추이를 살펴본 전향적 추적연구이다. 주요 결과는 다음과 같다. 수술 전과 수술 일주일 후, 그리고 수술 3개월 후에 측정한 모든 MMSE-DS 측정치에서 여성노인 집단이 남성노인 집단에 비하여 비교적 낮은 점수를 보였다. 남성노인 집단과 여성노인 집단 간의 수술 일주일 후 MMSE-DS 측정 점수는 통계적으로 유의한 차이가 나타났다. 남성노인 집단의 경우는 수술 전의 측정 점수에 비하여 수술 일주일 후의 점수가 낮았고, 수술 일주일 후의 점수에 비하여 수술 3개월 후의 점수가 낮아지는 패턴을 보였다. 수술 전의 MMSE-DS 점수에 비하여 수술 3개월 뒤의 MMSE-DS 점수가 통계적으로 유의하게 낮은 것으로 나타났다. 남성노인은 수술 후에 꾸준히 인지기능이 저하되는 결과를 확인할 수 있었다. 여성노인 집단의 경우는 수술 전의 측정 점수에 비해 수술 일주일 후의 점수가 통계적으로 유의하게 낮아졌다. 그러나 수술 3개월 후에 측정한 MMSE-DS 점수는 수술 일주일 뒤에 측정한 점수에 비하여 유의하게 높아지는 결과를 확인할 수 있었으며 수술 전에 측정했던 MMSE-DS 점수와 통계적으로 유의한 차이가 없는 수준으로 회복하는 것으로 나타났다. 본 연구는 여성노인과 남성노인의 수술 경험에 따른 인지기능의 영향을 알아보고 이를 성별에 따라 분석함으로써 노인의 인지기능에 대한 이해와 인지저하를 예방하고 향상시키기 위한 임상적 개입 방안에 대한 함의를 논의하였다.

Abstract

This study was performed using MMSE-DS, which is widely used to measure cognitive ability, to compare cognitive functions of elderly patients before and after hip arthroplasty according to gender. This study was performed in 44 patients and underwent hip surgery. MMSE-DS was repeated three times (before surgery, one week after surgery, and three months after surgery), and the changes were examined. The main results are as follows. In all MMSE-DS measurements, the female elderly group showed lower scores than the male elderly group. There was a statistically significant difference in the MMSE-DS scores between the male elderly group and the female elderly group after one week of operation. In the male elderly group, the score after surgery one week was lower than the score before surgery, and the score after 3 months was lower than the score after surgery one week after surgery. The MMSE-DS score at 3 months after surgery was statistically significantly lower than the MMSE-DS score before surgery. The male elderly were able to confirm the result that the cognitive function was decreased steadily after the operation. In the female elderly group, the score of the postoperative one week was significantly lower than the preoperative score. However, the MMSE-DS score at 3 months postoperatively was significantly higher than that at 1 week postoperatively, and the MMSE-DS score before surgery remained at a level that was not statistically significant appear. The results of this study suggest that the time for intervention should be different for male elderly and female elderly. Elderly women need active intervention to maintain cognitive function immediately after surgery. On the other hand, in the case of the male elderly group, a long - term and steady cognitive restoration program should intervene to prevent deterioration of cognitive function. The purpose of this study was to investigate the effects of cognitive function according to the surgical experience of female elderly and male elderly and to analyze according to gender differences to understand the cognitive function of the elderly and to discuss the implications of clinical intervention to prevent and improve cognitive decline.
