Abstract

(Background and Purpose) Competition among hospitals is intensifying as the supply of healthcare services exceeds demand. Therefore, many medical institutions are committed to improving patient satisfaction, and shortening the duration of treatment is becoming a significant factor in service satisfaction. However, hospitals need to achieve maximum effectiveness with limited resources, thereby making it difficult to shut down their systems for long periods to improve facilities. Moreover, the risk in terms of cost is very high without effective forecasts or predictions. Therefore, this study presents a service optimization analysis method that considers the providers and consumers of healthcare using discrete event simulation. (Method) Discrete event simulation analysis was applied, based on space markup modeling to extract resource (supplier) and entity (consumer) variables. Indices, such as the queue size for each examination, examination processing time, time required to examine each person, distribution of examination time, and utilization of resources, were designed, and a simulation environment was built to extract the indices by time. After identifying the cause of service processing delays at an actual examination center, an empirical study was conducted to derive an optimal solution through the reallocation of resources and relocation of examination rooms. (Results) Intuitive preliminary simulations showed that the queues increased significantly for electrocardiogram, pulmonary function, and ultrasound, in that order. Moreover, the utilization of resources was also in the order of ultrasound, pulmonary function, endoscopy, and electrocardiogram. After reallocating the resources of anthropometric measurement, blood-gathering, and ophthalmology with low resource utilization to examination rooms with high resource loads, the simulation results showed that the total processing time (working hours) decreased by 40.66%p on average, and the average examination time required for each person decreased by 43.82%p. Reallocating resources and relocating some examination rooms with easy-to-move medical equipment reduced the congestion of traffic lines and improved spatial cognition significantly. (Conclusions) The main findings are as follows: 1) The occurrence of queues due to resource overloads was the main cause for the decrease in service efficiency. 2) Distributing resource loads to prevent the concentration of queues on specific services is the most effective way to reduce the examination time. 3) The process of first calculating the number of services through preliminary simulations and determining the size of the space according to the number of services is reasonable. 4) Relocating examination rooms by analyzing the network between these rooms reduced the congestion in traffic lines significantly. 5) The length (distance) of the traffic lines is less affected by the time delay compared to the generation of queues; thus, space markup modeling can produce fairly close predictions if the required rooms are determined, even if the layout and shape of the space are not confirmed. Further research may become a useful tool in the space programming process.
