외상성 이개 손상의 치료

초록
이개는 외부로 돌출 및 노출되어 있어 안면외상에서 큰 부분을 차지한다. 외상성 이개 열상은 다양한 임상 양상으로 응급실을 방문하게 되는데 초기에 적절한 처치를 하지 않으면 다양한 후유증을 남길 수 있고 반복적인 수술을 해야 할 수도 있기 때문에 초기 치료가 중요하다. 하지만 외상성 이개 열상은 피부나 연골 손상 정도, 침범 부위에 따라 다양하기 때문에 정형화되고 통일된 치료 원칙을 적용하기 어렵다. 또한 이개는 그 모양이 오목, 볼록한 면들이 동시에 존재하고 얇고 약한 연골이 곡선 모양으로 이루어져 있으며 전면부는 피부와 연골이 단단히 서로 유착되어 있어 조작이 힘들어 수술 후에 이개의 변형이 남을 가능성이 높다. 따라서 외상으로 인한 이개 손상을 치료할 때 술자들은 반드시 이개의 해부학적 특성을 이해하고 여러 가지 수술기법을 적용할 수 있어야 미용적으로 만족스러운 결과를 얻을 수 있을 것이다.
