The authors report one case of carotid artery rupture in a 51-year-old woman, which occurred during a tracheostomy with successful treatment by bovine patch angioplasty. In order to avoid various complications of tracheostomy, risk factors such as obesity, short neck and bleeding tendency must be evaluated before tracheostomy. Elective tracheostomy by trainee might be better to be performed under general anesthesia with intubation during daytime when an experienced staff can help in emergency. During tracheostomy, continuous identification of midline trachea with palpation is crucial to avoid injuries in surrounding structures.
