약품비 지출 목표 설정을 통한 가격-사용량연동제 개선 방안 연구

일부 국가에서 약제비 절감 방안의 일환으로 지불자와 제약사 간 약가결정에서 재정에 근거한 합의(Finance-based schemes) 방식의 위험분담 방식을 활용하고 있다. 우리나라의 경우 국민건강보험공단에서 2007년부터 약가협상지침에 근거하여 제약회사와 의약품의 가격-사용량 등을 고려하여 약가협상을 수행해 왔다. 실제 2007년부터 2009년 2/4분기 동안 가격-사용량 규제 대상 의약품에 가격조정 산식을 적용한 결과 예상사용량 대비 실제사용량 증가율은 68%～487%로 증가폭이 매우 크나 약가협상 참고산식을 적용한 결과 약가조정률은 -4.1%～-8.3%에 불과한 것으로 나타났다. 이는 실제사용량이 예상사용량을 크게 증가하더라도 약가 인하는 참고식에 근거하여 10% 미만으로 되어 가격변동 폭이 크지 않다고 판단하여, 제약사가 과소 추정된 예상사용량 자료를 제출하는 것으로 판단된다. 이 글은 현행 가격-사용량 연동제 제도의 문제점을 고찰하고, 약품비지출 증가목표치를 산출하여 이를 기반으로 한 재정근거 합의방안의 개선 방향을 제시하고자 하였다. 가격-사용량연동제는 재정위험 보전 기능을 강화할 필요가 있으며, 이를 위해 의약품 효능군별로 약품비지출 증가목표치를 설정하여 적용할 필요성이 있다. 또한 현행 가격-사용량 연동제는 초과사용량 허용기준을 줄이고, 초과 지출금액 상한을 설정하는 방식을 도입하는 등 보완이 필요하다.
Abstract
'Finance-based risk sharing schemes' between payor and pharmaceutical industry is commonly used in many countries to contain pharmaceutical expenditures. In Korea, finance-based schemes launched as forms of price-volume agreement for newly listed drugs since 2007 when the National Health Insurance Corporation and pharmaceutical industry negotiated listed drug prices and demonstrative refund system for orphan drugs since 2009. The growth rates of real volume over expected volume estimates of newly listed drugs in positive list system (from 2007 to Second Quarter of 2009) reached to 68%～487%, which showed significant variance, but range of price decrease rate were only -4.1%～-8.3%. This fact means that even if pharmaceutical industry report low expected volume but the real volume increase highly, which result price decrease less than 10%. It implicates that pharmaceutical industry inclines to report under estimate of expected volume. This article aims to suggest improving price-volume arrangement system by linking expenditure target for pharmaceutical price negotiation. Authors recommend as follows. First, the pharmaceutical expenditure target need to be placed to avoid financial risk and applied to price-volueme arrangement for the therapeutical groups. Second, the current arrangements need to be re-designed into decreasing the permissible range of excess volume, and setting the cap amount of excess expenditure and the recovery.
