장기요양 치매와 비치매 노인의 건강상태, 서비스 욕구와 급성기 의료이용의 관련요인

우리나라 치매 유병률은 급속히 증가하고 있으며, 노인장기요양보험 이용자의 상당수가 치매를 앓고 있음에도 불구하고 이들의 서비스 니즈와 이용에 관한 연구는 아직까지 제한적이다. 본 연구의 목적은 노인장기요양보험 하의 치매노인들의 건강상태와 서비스 욕구를 신뢰·타당도가 검증된 포괄기능평가도구를 통해 수집한 자료를 이용해 보다 상세히 살펴보고, 이러한 니즈와 의료이용의 관계를 비치매 노인과 비교하여 살펴보고자 하는 것이다. 연구대상자는 요양시설 및 재가기관에서 장기요양서비스를 이용하는 치매와 비치매 노인 총 571명을 대상으로 하였으며, 이들의 건강상태와 서비스욕구를 한국어판 인터라이 장기요양기관 및 홈 케어 평가도구(interRAI LTCF & interRAI HC)를 이용하여 파악하였다. 연구결과, 조사대상 노인의 45.70%가 치매였으며, 의사소통능력과 인지기능 및 문제행동에서 비치매군과 차이가 있었고, 서비스 욕구에서도 두 군 간에 유의한 차이가 있었다. 급성기 의료이용 서비스 이용여부는 서비스욕구수준, 서비스이용세팅 및 이용기간과 관련이 있었다. 본 연구는 한국 장기요양이용 치매 노인군과 비치매 노인군을 비교하여 이들 간에 건강상태와 서비스욕구에 차이가 있고, 임상적 서비스 욕구, 장기요양이용 세팅, 이용기간이 의료이용에 주요한 영향요인임을 실증자료를 통해 최초로 보고한 연구이다. 향후 대표성 있는 자료를 이용하여 치매노인의 서비스 니즈와 의료이용 관련요인을 다면적, 종단적으로 살펴보는 연구가 필요하다.

Abstract

Korea’s population is rapidly aging, and a large portion of the beneficiaries of public long-term care insurance are older people with dementia; yet there is limited evidence on the care needs and service use of this vulnerable population. Though it has only been conducted to a limited extent, comprehensive assessment is invaluable for understanding health status and service needs of long-term care beneficiaries with dementia. This study is to examine the relationships between health status, care needs, and service utilizations among long-term care residents with and without dementia. This study compared health status, care needs, and service utilizations of older people with and without dementia in nursing home (NH) and home care (HC) settings. A total of 571 older people were examined using interRAI LTCF and interRAI HC. Two-prat models were adopted. Results showed that of the subjects, 45.70% had dementia, and they were more likely to use NH rather than HC service, compared to those without dementia. The dementia group had a higher number of service needs, measured by CAPs, and behavior problems were also higher than among their counterparts. Several factors, including clinical care needs and service settings, length of stay, were associated with health service utilizations of long-term care beneficiaries. This study presented significant differences in health status and service needs between dementia and non-dementia groups. Dementia patients had high care needs related to functional performance and cognition/mental health. Better quality of dementia care is needed in long-term care settings in Korea.
