Abstract

The ‘value of hope’, introduced by the ISPOR Special Task Force in 2018, refers to patients' potential preference for treatments offering long-term survival prospects despite risks and uncertainties. This qualitative study explored this concept through semi-structured, in-depth interviews with 10 individuals diagnosed with serious illnesses (8 cancer, 2 congenital heart disease) and 10 general public participants from August to September 2023. The study investigated considerations in treatment choices and preferences for uncertain outcomes across various scenarios. Both groups tended to prefer risky options offering potential long-term survival, with this trend more pronounced in the patient group. Patients emphasized ‘hope’ and ‘goals’ associated with long-term survival as essential for maintaining a meaningful life and positive attitude in their present lives. While the general public expressed vague fears about treatment side effects, patients generally viewed them as challenges to overcome, demonstrating greater acceptance. Preferences were influenced by various factors, including the nature of risky alternatives, minimum survival periods, and survival differentials compared to risk-free options. The study also observed a stronger inclination towards risky options for younger hypothetical ages. Although this study confirms the presence of the ‘value of hope’, it underscores its complexity and the influence of various factors, emphasizing the need for careful consideration in its integration into health technology assessments. Despite the limitation of a small sample size, this qualitative research promotes an in-depth understanding on the ‘value of hope’ and provides significant insights for future large-scale studies.
