Abstract
Osteoporosis is clearly a serious public health problem in the general population, resulting in considerable morbidity and mortality. Preliminary studies indicate that psychiatric patients are at elevated risk for osteoporosis and fragility fractures ; possible factors include lack of exercise, poor nutritional status, cigarette smocking, alcohol drinking, medications. Obesity could play a protective role. Of particular concern is the impact of antipsychotic medications that produce hyperprolactinemia. The patients treated long-term with agents that elevated prolactin sufficiently to suppress estrogen levels are at heightened risk for osteoporosis. Although the effects of antipsychotics on prolactin have been well studied, effects upon estrogen and testosterone have received little attention despite the important implications for bone mineral density. Clinicians should address with their schizophrenia patients general health issues such as cigarette smocking, diet and medication, witch may contribute to osteoporosis. The loss of menses in a woman treated with prolactin-elevating medication should raise the concern of hypoestrogenism and should be given a test for bone mineral density. The author reviewed osteoporosis and related risk factors in schizophrenia patients for prevention and early treatment for osteoporosis.
