초록

자살을 시도하는 환자는 정신과적인 응급처치를 필요로 한다. 우리는 여기서 자살 환자를 치료하는데 있어서 정신치료적인 접근법과 약물치료 방법, 청소년과 노인의 자살에 대한 접근법과 예방법을 살펴보고, 자살 환자를 흔히 접하게 되는 내과 의사와의 상호 협력과 팀치료 접근법을 살펴보았다. 자살 시도 환자에게 도움이 된다고 알려진 정신치료적 접근법으로는 인지행동치료가 우울증 환자에게 도움이 되고, 변증법적 치료는 인격장애 환자에게 효과적이다. 자살 환자의 약물치료는 급성 스트레스에서는 benzodiazepine계 약물을 사용하고, 만성적인 환자들에는 세로토닌 선택적 재흡수 차단제들, lithium, 항정신병약물 등을 사용한다. 청소년 자살 환자의 치료에서는 부모의 협조가 매우 필요하고, 노인 환자의 자살예방과 구심성 관리 프로그램에 대해 살펴 보았고, 자살 환자의 진료를 담당하는 일차진료의사에 대한 지속적 교육 훈련이 필요하다는 것을 살펴보았다.

Abstract

Suicidal patients need a psychiatric emergency care. We discuss and review the psychotherapeutic managements, psychopharmacological treatments for suicidal patients, therapeutic approaches for the adolescent and elderly suicidal patients and collaborative treatment of psychiatrists and primary care physicians, herein. Cognitive-behavioral treatment is helpful to the depressed patients and dialectical- behavioral treatment to borderline personality disorder patients. In the psychopharmacological managements of suicidal patients, benzodiazepines are used to the acute stressful patients and SSRIs, lithium, neuroleptics to the chronic patients. Parent’s cooperation is necessary to adolescent suicidal treatment. We discuss the Tele-help/Tele-check program in suicidal elderly treatment and collaborations between psychiatrists and other physicians.
