범물안장애환자와 주요우울증환자 간에 인지적 형태와 스트레스 대처 방식의 비교

본 연구는 유전적 공통성과 높은 병발율을 보이는 범불안장애와 주요우울증환자의 발병 전 에 가지고 있던 인지적 요소를 비교 분석하고자 하였다. 이를 위해 부산 봉생 병원에 외래 통원중인 환자를 대상으로 DSM-Ⅳ진단기준에 따라, 2명의 정신과 전문의에 의해 기타 질환이 동반되지 않은 범불안장애, 주요우울증으로 진단된 환자 가운데서 설문 가능한 범불안장애 환자 10명, 주요우울증 환자 16명 총 26명의 환자에게 얻은 결과를 최종 분석 대상으로 하였다.우울척도, 불안척도, 역기능적 태도척도, 스트레스 대처방식척도, 비합리적 신념척도, 인지도식 척도를 한 벌로 묶은 설문지를 사용하여 자기 보고형으로 검사하였다.범불안장애 환자와 주요우울증 환자의 첫 증상 발생당시의 불안, 우울 정도와 그리고 증상 발병 이전 평소에 가지고 있던 역기능적 태도, 스트레스 대처방식, 비합리적 신념, 인지도식 등의 차이를 비교한 결과 다음과 같이 요약할 수 있다.1) 주요우울증군의 우울 정도는 범불안장애군보다 높았고 범불안장애군의 불안정도는 주요 우울증군보다 높았다2) 범불안장애군와 주요우울증군 간의 발병 전 역기능적 태도정도에는 유의한 차이가 없었 다.3) 발병 전 스트레스 대처 방식 비교에 있어서 주요우울증군은 범불안장애군에 비해 동화 (양보) 척도의 점수가 유의하게 높았다..4) 범불안장애군과 주요우울증군 간의 발병 전 비합리적 신념척도에서는 유의한 차이가 없 었다.5) 발병 전 인지도식(schema)비교에서는 범불안장애군이 주요우울증군에 비해 취약성 척도 가 유의하게 높았다.
Abstract
Objectives; This study was performed to compare the premorbid cognitive styles and stress coping strategies between generalized anxiety disorder and major depressive disorder which share same genetic factors with high comorbidity rate. Methods; The subjects were 10 patients with GAD and 16 patients with MDD. The questionaires of the scales for anxiety, depression, premorbid dysfunctional attitudes, irrational beliefs, schema and stress coping strategies were administered. Results; 1) The level of anxiety of GAD was higher than that of MDD, the level of depression of MDD was higher that of than GAD. 2) There was no differences of degree for the premorbid dysfunctional attitudes between two groups. 3) The group of MDD had higher score than group of GAD on accommodation subscale of the premorbid stress coping strategies. 4) There was no differences of degree for the premorbid irrational beliefs between two groups. 5) The group of GAD had higher score than group of MDD on vulnerability subscale of the premorbid schema. Conclusion; The premorbid cognitive styles and stress coping strategies between GAD and MDD were more or less different, but seemed to have similarity as a whole.
