망막대동맥류에 합병된 황반부종에서 유리체강내 베바시주맙 주입술의 치료효과

목적: 망막대동맥류에 합병된 황반부종에서 유리체강내 베바시주맙 주입술의 치료 효과를 알아보고자 하였다. 대상과 방법: 망막대동맥류 환자 8명 9안을 대상으로 전향적 연구를 시작하였다. 유리체강내 베바시주맙 주입술을 8주 간격으로 평균2.00 ± 0.67회 주입하였다. 시술 전, 시술 후 2, 4, 6개월에 시력검사, 빛간섭단층촬영을, 시술 후 3개월경에 형광안저혈관조영을시행하였다. 결과: 평균 나이는 73.6 ± 6.9세였고, 평균 관찰기간은 7.3 ± 2.6개월이었다. 최대교정시력은 주입술 전 평균 logMAR 1.92 ± 0.67에서 주입 후 평균 logMAR 0.87 ± 0.57로 호전되었으며 중심황반두께는 주입술 전 평균 388 ± 168 μm에서 주입 후 평균 200 ±39 μm로 감소하였다. 황반부출혈 유무에 따른 비교에서 황반출혈이 없는 3안의 술 후 시력이 통계적으로 유의하게 더 좋았다. 모든 안에서 주입 후 3개월 형광안저혈관조영상 형광누출이 없거나 감소하였고, 치료에 의한 부작용이나 합병증은 발생하지 않았다. 결론: 망막대동맥류에 합병된 황반부종의 치료로서 유리체강내 베바시주맙 주입술은 단기간 효과적이고 안전하였다.
Abstract
Purpose: To study the short-term effect of intravitreal injection of bevacizumab in the treatment of macular edema secondary to retinal macroaneurysm. Methods: Eight eyes of 9 patients who underwent consecutive intravitreal bevacizumab injections for macular edema secondary to retinal macroaneurysm were prospectively evaluated. Complete eye examination including the best corrected visual acuity (BCVA), fundus examination and optical coherence tomography (OCT) were performed at baseline and follow-up visits at 2, 4 and 6 months. Results: The mean age was 73.6 ± 6.9 years, and the mean study period was 7.3 ± 2.6 months. At the final visit, BCVA improved from log MAR 1.92 ± 0.67 to log MAR 0.87 ± 0.57. The mean central macular thickness on OCT image decreased from 388 ± 168 μm at the initial visit to 200 ± 39 μm at the final visit. Three eyes without subretinal hemorrhage presented better visual outcome than eyes with subretinal hemorrhage. All eyes showed some decrease in angiographic leakage. No adverse side effects were observed following the injections. Conclusions: Intravitreal bevacizumab injection can be used as a selective alternative treatment modality for macular edema secondary to retinal macroaneurysm.
