목적: 만성적이고 스테로이드 점안약에 의존적인 타이거슨 표층점상각막염 환자에서 상안검판 트리암시놀론 주사술로 장기간 재발을억제하였기에 보고하고자 한다. 증례요약: 내원 6년 전부터 반복되는 양안 충혈, 눈부심, 눈물흘림을 주소로 37세 여자 환자가 내원하였다. 세극등 검사상 양안 모두에서 다수의 융기된 점상 병변이 발견되었고 플루오신 염색약에 염색되는 양상이었으며, 그 깊이는 상피와 상피하까지 침범하는 양상이었다. 타이거슨 표층점상각막염 진단하에 0.1% fluorometholone, 0.05% cyclosporin, 0.15% ganciclovir를 처방하고 경과 관찰하였다. 10개월간의 경과 관찰 결과, 우안에는 2차례, 좌안에는 7차례 재발 및 호전을 반복하였으며 스테로이드 점안액에 의존성을 보였다. 이에 양안 모두 상안검판 트리암시놀론 주사술을 시행하고 인공눈물만 투약하였다. 주사 후 4개월 외래 경과관찰 기간 동안 각막염발현 없었고, 재발하는 소견 보이지 않았다. 결론: 만성적이고 스테로이드 점안약에 의존적인 타이거슨 표층점상각막염 환자에서 상안검판 트리암시놀론 주사술이 장기간 재발을억제하는 데 효과적인 방법이 될 수 있다.
Abstract
Purpose: To report a case of supratarsal injection of triamcinolone for the management of chronic, steroid-dependent Thygeson’s superficial punctate keratitis. Case summary: A 37-years-old woman complained of redness, photophobia, and tearing in both eyes, which lasted for 6years. The slit lamp examination revealed multiple intraepithelial and subepithelial, punctated corneal lesions which were elevated and scattered diffusely upon staining with fluorescein dye in both eyes. The patient was diagnosed with Thygeson’s superficial punctate keratitis and treated with 0.1% fluorometholone, 0.05% cyclosporin, and 0.15% ganciclovir in both eyes. After 10 months of follow-up, recurrences were reported twice in the right eye, and 7 times in the left eye. The patient was treated with a supratarsal injection of triamcilonone in both eyes and after injection, the patient was treated only with artificial tears. Four months later, the patient did not complain of any symptoms and her cornea was clear. Conclusions: A supratarsal injection of triamcinolone may be an effective method to prevent recurrence of chronic, steroid-dependent Thygeson’s superficial punctate keratitis.
