목적: 수술일 입원하여 23게이지 유리체절제술을 받고 당일 퇴원한 환자들의 임상경과를 관찰하여, 당일 23게이지 유리체절제술 후합병증 발생률을 알아보고자 하였다. 대상과 방법: 2009년 9월부터 2010년 9월까지 당일 23게이지 유리체절제술을 받고 3개월 이상 경과관찰이 가능했던 79명 79안의의무기록을 후향적으로 분석하여, 수술 후 시력 및 안압의 변화와 합병증의 발생 등을 조사하였다. 결과: 평균 logMAR 최대교정시력은 수술 전 1.32 ± 0.96에서 수술 후 3개월에 0.40 ± 0.41으로 호전되었고(p˂0.05), 평균 안압은수술 전 14.00 ± 3.57 mmHg에서 수술 후 1일에 15.05 ± 6.63 mmHg으로 유의한 변화는 없었으며(p˃0.05), 수술 종료 시 공막절개창누출로 봉합이 필요한 경우가 4안(5.1%)이었다. 수술 후 합병증은 10안(12.7%)으로 안압 상승 7안(8.9%), 저안압 2안(2.5%) 및 유리체출혈 1안(1.3%)이었고 재수술이 필요한 경우는 없었다. 결론: 당일 23게이지 유리체절제술을 시행 받고 퇴원한 환자들에서도, 수술 후 심각한 합병증 없이 시력의 호전을 얻을 수 있었다.
Abstract
Purpose: To evaluate the complication incidence of 23-gauge pars plana vitrectomy day surgery. Methods: A retrospective review was conducted on 79 eyes that underwent 23-gauge pars plana vitrectomy day surgery from September 2009 to September 2010. The main outcome measures included changes in best corrected visual acuity (BCVA), intraocular pressure (IOP), and presence of postoperative complications after the day surgery. Results: The mean preoperative log MAR BCVA improved from 1.32 ± 0.96 to 0.40 ± 0.41 at 3 months after surgery (p <0.05). There was no significant differences between the mean preoperative IOP and IOP on the 1st day after surgery (p >0.05). At the end of surgery, suture placement was required for sclerotomy site leakage in 4 eyes (5.1%). There were 10eyes (12.7%) with postoperative complications, including 7 eyes with increased IOP (8.9%), hypotony in 2 eyes (2.6%) and vitreous hemorrhage in 1 eye (1.3%), with no case requiring reoperation. Conclusions: The patients who underwent 23-gauge pars plana vitrectomy day surgery achieved visual improvement without serious complications postoperatively.
