레스토Ⓡ 다초점 인공수정체 삽입술 후 잔여굴절이상에 대한 레이저 굴절교정수술 효과

초록

목적: 레스토Ⓡ 다초점 인공수정체 삽입술 후 발생된 잔여 굴절이상을 엑시머레이저 굴절교정수술로 교정한 후 그 효과를 알아보고자하였다. 대상과 방법: 본원에서 수정체 초음파 유화술 및 레스토Ⓡ 다초점 인공수정체 삽입술을 시행 받은 환자를 후향적 의무기록을 통해 분석하였다. 잔여 굴절이상이 있는 6명 10안에서, 5안은 PRK, 5안은 LASIK을 시행 받았다. 결과: 백내장 수술 이후 굴절교정수술 전, 평균 구면렌즈 대응치는 -0.34 ± 0.89D, 평균 난시는 2.08 ± 0.55D였다. 술 후 6개월에평균 구면렌즈 대응치는 -0.03 ± 0.30D로 감소하였으나 통계적으로 유의하지는 않았고(p=0.354), 평균 난시는 0.23 ± 0.28D로통계적으로 유의하게 감소하였다(p˂0.0001). 원거리 나안시력이 술 후 6개월에 0.11 ± 0.10 (logMAR)으로 술 전에 비해 통계적으로유의하게 호전되었으나(p˂0.0001), 원거리 최대 교정시력과 근거리 나안시력의 호전은 없었다(p=0.073, p=0.100). 결론: 레스토Ⓡ 다초점 인공수정체 삽입술 후 잔여굴절이상 발생시 엑시머레이저를 이용한 굴절교정수술은 예측성, 안정성 측면에서임상적으로 유용한 방법이다.

Abstract

Purpose: To evaluate the clinical effects of excimer laser refractive surgery on eyes with residual refractive error after implantation of ReSTOR® multifocal intraocular lenses. Methods: We retrospectively reviewed the medical records of 10 eyes of 6 patients who had undergone cataract surgery with implantation of multifocal intraocular lenses. Photorefractive keratectomy (PRK) (5 eyes) and laser-assisted in situ keratomileusis (LASIK) (5 eyes) was performed on 10 eyes with residual refractive error. Results: After cataract surgery and before laser surgery, the mean spherical equivalent (SE) was -0.34 ± 0.89 D and the mean astigmatism was 2.08 ± 0.55 D. Six months after laser surgery, the mean SE was -0.33 ± 0.30 D (p = 0.354) and the mean astigmatism was 0.23 ± 0.28 D. The changes in astigmatism between paired preoperative and postoperative values were statistically significant (p < 0.0001). At 6 months after surgery, uncorrected distance visual acuity significantly improved to 0.11 ± 0.10 (log MAR) (p < 0.0001) There were no significant changes in the best corrected distance visual acuity and uncorrected near visual acuity (p = 0.073, p = 0.100). Conclusions: On the basis of predictability and stability, excimer laser surgery appears to be a clinically useful procedure to correct residual refractive error after implantation of ReSTOR® multifocal intraocular lenses.
