목적: 오노디 세포내 곰팡이 덩이에 의한 시신경병증 1예를 보고하고자 한다. 증례요약: 63세 여자가 1주 전에 재발한 우안 시력저하와 통증으로 진료 의뢰되었다. 2주 전에 우안 시력저하로 방문한 타 병원에서우안 시신경염으로 진단받고 고용량 스테로이드 투여 후 호전되었다가 1주 전에 재발하였다. 타 병원 진료의뢰서에 의하면 뇌자기공명영상에서 다발성 경화증을 의심할 소견은 없으며 코곁굴내 약간의 염증 외에는 특별한 소견은 없다고 하였다. 초진 시 최대교정시력은우안은 안전수지 10 cm, 좌안은 1.0 이였고, 우안에서 상대적 구심성 동공장애가 관찰되었다. 안저검사에서 우안의 시신경 유두부종이관찰되었고 시야검사에서 우안은 완전 시야결손을 보였고, VEP검사에서 우안에서 P100이 나타나지 않았다. 재발된 시신경염으로 진단하고 고용량 스테로이드 투여 후 유두부종은 감소하고 시력은 0.6으로 호전되었으나, 2주 후 다시 우안 시력이 0.4로 악화되었다. 외부 뇌자기공명영상 확인 결과 우측 오노디 세포내 곰팡이 덩이에 의한 시신경병증이 의심되어 이비인후과에서 곰팡이 덩이 제거수술을 받았다. 수술 3주 후 시력은 0.9로 회복되었고, 안저검사에서 시신경 유두부종은 관찰되지 않았으며, VEP검사에서 정상 소견을 보였다. 결론: 시신경염이 의심되는 환자에서 오노디 세포내 병변 또한 감별해야 하며 필요한 경우 영상학적 검사로 진단하고 시력회복을 위해신속하게 수술적 제거를 해야 한다.
Abstract
Purpose: To report a rare case of optic neuropathy caused by a fungal ball in an Onodi cell. Case summary: A 63-year-old female was referred to our clinic with relapsed visual loss and ocular pain in the right eye. She had been diagnosed as optic neuritis 14 days before and given pulse steroid therapy. She recovered to normal but relapsed7 days before. In medical referral there was no suspected multiple sclerosis but only a few inflammation in the paranasalsinuses. On our initial examination, best corrected visual acuity was counting finger at 10 cm in the right eye, and 1.0in the left eye, along with relative afferent pupillary defect in the right eye. The fundoscopic examinations disclosed discswelling; nearly total visual field defect was observed on visual field examination and visual evoked potential test revealeddecreased amplitude at P100 wave in the right eye. Clinical impression was relapsed optic neuritis. After the administrationof pulse steroid therapy, her disc swelling was decreased and visual acuity was recovered to 0.6, however, visual acuitywas exacerbated to 0.4 in 2 weeks. We checked outside brain magnetic resonance imaging (MRI) and the result showedoptic neuropathy caused by a fungal ball in an Onodi cell. The patient was referred to otorhinolaryngologist and fungal ballwas removed by endoscopic sinus surgery. 3 weeks after surgery the patient’s visual acuity was 0.9, no disc swelling wasfound and visual evoked potential was recovered to normal. Conclusions: An Onodi cell lesion should be considered in the differential diagnosis of optic neuritis, identified by imagingstudies and promptly removed by surgery for visual recovery.
